Costume Collection

Interested in having a friend or family member collect your costume on your behalf? If so, please complete the form below.

Please Note:

• The authorized person will be allowed to collect your costume package.
• The authorized person must present a valid government issued photo identification (ID) which includes a driver’s license, a state issued photo ID, or a passport.
• All costumes must be paid in full before collecting. Absolutely NO exceptions.
• Karibbean Vibrationz will not be held responsible for any inaccuracies in the information you have provided.
• Karibbean Vibrationz will not be held responsible for any arrangements made between you and the person authorized to collect your costume.
• Karibbean Vibrationz will not be held responsible for any theft, fraud, damages or losses incurred once the costume had been collected by your authorized person.
• Costumes remain the property of Karibbean Vibrationz until paid for in full and collected.
